How to Apply for a Therapeutic Riding Program

If you would like your child to participate in a therapeutic riding program, then the first step is to get a referral from your physician. This also applies if you yourself wish to participate. You can download a Physician Referral Form for your physician to complete. There is also a document giving Guidelines For Physicians/Therapists, which they will find useful. These are PDF files that are readable with Acrobat Reader, which is free software.

 

VTEA Adobe Reader

If you need Adobe Reader, you can download it at the Adobe Download website. The initial choice of Operating System at this site is Windows XP. If you have another operating system, then you must select it.

 

You must also complete and sign two further forms you can download. These are the Client Information Form and a Parent/Guardian Release Form.

 

You should then send or give the forms to Julie Matijiw, Head Trainer, who can arrange an appropriate assessment session. Please be aware that the demand is high so it may not be possible to set up a therapeutic riding program immediately in all cases. You can explore the current situation by contacting Julie.
